Our Case Managers work directly with our clients
Heather qualified as an Occupational Therapist (OT) in 1989 and worked for the NHS for 18 years both as an OT and Brain Injury Co-ordinator. She has been case managing for many years opening her own company Total Case Management Limited (TCM) in 2008. Heather is a highly experienced Case Manager and is registered with the Health Professions Council, is a member of the British Association of Occupational Therapists (BAOT) and British Association of Brain Injury Case Managers (BABICM).
Louise is a registered nurse (adult) and health visitor with over 20 years experience working with children and adults. She has significant experience working with adults and children who have an acquired brain injury leading to complex care needs either through accident or birth trauma. Her specific clinical skills include tracheostomy, long term ventilation, gastrostomy, colostomy, epilepsy, limited mobility including quadriplegia and tetraplegia, and cerebral palsy. Louise is an autonomic dynamic practitioner who can work effectively in a team including working collaboratively with other health and social care professionals. Louise has excellent communication skills and can relate to clients, families and liaise with other health professionals to achieve the best possible outcomes for our client's. Louise is registered with BABICM.
Hazel is a registered Social Worker, qualifying in 2014. Recently she qualified as a Best Interests Assessor (BIA), a specialist role identifying whether a deprivation of liberty is occurring.
During her career she has worked collaboratively in placing children for adoption who have suffered trauma. The consequences of which have required therapeutic intervention. Hazel has extensive experience in completing care plans to reflect the needs and support required. Hazel had to regularly gain insight and further medical evidence as required from healthcare professionals and services, to form assessments. She was also an appointed adoption panel member.
Hazel has experience of working in care management and assessment teams. She has working knowledge of child protection procedures and the threshold of social care involvement. Experience includes undertaking initial and core assessments, managing direct payments, chairing meetings, and liaising with professionals in a multi-agency capacity.
Hazel has experience of working within community mental health and learning disabilities teams. She has also worked for a disability rights charity, supporting those with a brain injury and/or physical injuries. She represented individuals at tribunal who had been unfairly assessed as part of their benefits claims.
On a personal level, Hazel has lived with the consequences of a brain injury having grown up with a family member with an ABI. The family have been involved with Headway for many years.
Sebastien joined Total Case Management in November 2022 as a Brain Injury Case Manager. He initially qualified as an Occupational Therapist in 2017, and worked in the NHS for over 5 years.
Seb brings to his role as a case manager a range of experience working with clients who have acquired brain injury as well as experience of working with clients who have suffered a variety of orthopaedic injuries.
Seb has spent time as a therapist working within community and acute settings, as well as longer-term rehabilitation units. He has worked with a variety of other healthcare and social care professionals to assess and plan treatments for complex clients. The experience he gained in the NHS and community ensures his clients, their family, the multi-disciplinary team and care team have guidance and support. He is driven to ensure the client achieves the best possible outcomes.
Seb is an autonomous professional with excellent communication skills and takes holistic approach.

Collette has worked in health and social care for the last 25 years and has managed complex services and teams, she has a registered managers award and will be applying to the Care Quality Commission to be the registered manager for TCM in the coming months. This role will entail being responsible for maintaining the quality of service provided by Total Case Management Ltd.
Collette’s previous experience of working as a Senior Team Leader makes her the perfect person to support the care staff who are managed by TCM ensuring their mandatory training, skills and knowledge are always up to date, Collette also carries out all supervisions and PDR’s for these teams.
